		<description>I just found this info.  Great!  Question:  How do you create  boxes that are selectable within the template?  I was able to create a form in MS Word and once locked, you can check the boxes off as needed, when interviewing someone.  It&#039;s quick.  I would like to be able to do that with Pages.  Would appreciate any info on this.</description>
